Connect Plus has been developed alongside NHS clinicians and patients to make it easier for you to manage your health at home and in one place.
Users of Connect Plus select their local healthcare organisation from the dropdown menu and find clinically assured and regularly updated information on a wide range of conditions and procedures*.
Connect Plus will enable you to become more knowledgeable about your condition(s), monitor your symptoms, manage your medications and connect you to your local healthcare team.

Step by Step Guide
Once you have downloaded the Connect Plus app, open the app up and it will first ask if it can send notifications/ reminders.
You will want these notifications so press the “Allow” button.
Next, it will ask you to agree with the “Terms and Conditions”, to proceed to the next step.
Next, select which condition you have:
- Diabetic Foot
For this i will use “Rheumatology” as an example.
Once you have chosen the condition, you will have to log in by pressing the button in the top right.
Once the button has been pressed, you will be able to log in. If you do not have an account please press the “Create account” link under the “Log in” button.
Once you have pressed the link, you will have to enter in the details below and press “Create account”.
Then you will recieve an email from Connect plus to verify your email, once you have recieved the email please press the “Verify your account” button to proceed to the next step.
Once your email has been verified you will have access the the app. to go back to the main screen press the home button at the bottom of the screen.
Once the Home button is pressed, it will take you to this screen here, where you can add medication, appointments and much more.
