Individuals can self-refer to this service.

How to self-refer online

You can refer yourself to TALKWORKS for free online by visiting their section on the Devon Partnership Trust (DPT) website via self-referral

The short guide below shows this process:

1. Open your web browser and type ‘TALKWORKS’ in your search engine.

2. The TALKWORKS Depression Anxiety Service self-referral service  is on the Devon Partnership Trust or DPT website. Select the search result on the list which says ‘Self Referral Form I DPT’.

3. This will take you to a secure online form where you can register for the TALKWORKS Depression and Anxiety Service by clicking in each box and typing in your answers or clicking the answer button under each question.

Your Information is sent encrypted and is not accessible to anyone outside of those services. When you have finished filling out each page, click the yellow ‘NEXT’ button in the bottom right corner.

4. When you are happy with your answers, click the yellow ‘Submit’ button at the bottom right hand side of the page.

What happens next?

The TALKWORKS Depression and Anxiety Service will then contact you by phone or by email if you have selected YES to being sent information in this way, to arrange an appointment.

If they are unable to contact you or they do not hear from you again within 14 days, they will assume, at this time, you do not want an appointment and we will make no further contact. However, they are happy to accept a referral from you again at any time in the future.

If you are 16 years and over and registered with a Plymouth GP there are other ways that you can access the different services that Plymouth Options provide.

You can self-refer online by completing this online referral form

It is a very similar process to that above.
